Permission slip, Rules and Payment Dates

Williamsburg Trip 2018

Below are the highlights from the permission form including reasons for dismissal, payment schedule, and medication policies.  Please find the complete form form under forms.

 

 

Graham Middle School Reasons for losing access to the trip:

1. Four or more days of out of school suspension

2. Miss more than fourteen days of school (regardless of whether they are excused or unexcused absences)

3. poor academic performance (including inconsistency in completing classwork or homework assignments)

4. excessive tardies

5. consistently misbehaving

6. being disrespectful to fellow classmates or teachers.

*For items 3-6, a warning will be issued to the student and the parent will be notified before removal is considered. Removal will result in the loss of the non-refundable $95 down payment. There will be no refunds of any amount after April 13.

 

 

 

$95 deposit is due November 8 Schedule for $75

Payments:

January 8

February 5

March 5 

April 6

A missed payment will result in removal from the trip.

 

Medical Policies:

Medication must be registered by April 16 The medical information sheet that was in your child’s school calendar will be used for emergency situations, should any arise. If any changes to that form are necessary, please let the school nurse know immediately. No medication including items such as Tylenol, inhalers, or prescription medication will be allowed on the trip without proper documentation. Prescription medicine must also be accompanied by doctors orders. Please note, non-prescription medicine must still have a school form filled out in order to be allowed on the trip. closer to the trip date, we will be asking for a complete list of phone numbers where parents/guardians may be reached in case of emergency.
